Very condescending and rude. He cancelled my appointment and took my money without doing the exam because I forgot to fill out the MedExpress form that I hadn't filled out in five years. Despite coming in early for my appointment he refused to see me and didn't work with me at all even though it takes all but 3 minutes to fill out the MedExpress form. Instead he took the entire $230 that I paid for the medical and said that I had to reschedule and pay again if I wanted to get my medical with him. He was very rude and claimed that his receptionist sent me a reminder to fill out the MedExpress before the appointment, but I never received an email from their office, I only received an email to make the payment.
